The application has been designed to be simple and specific because Mark would have wanted it that way. Please include with your application a brief essay which should respond to the web page information and the intentions of the scholarship.

The application should include:

1. Copy (or copies, if undecided on where you will be attending) of the letter of acceptance from the college or university you will attend in September 2009
2. Color photo to be released to the media and posted on the Web site (write name on back of photo)
3. Brief essay
4. Mail complete application (no electronic submissions) to:

The application must be postmarked by May 1, 2010 to be considered by the committee.
